We are proposing to conduct a pilot single center (Hopkins) randomized controlled trial to obtain preliminary data on the feasibility, safety, and get potential evidence of efficacy of the goal-directed perfusion protocol (GDPP) for reducing the incidence of CSA-AKI. We will use data from pilot RCT to prove that we can enroll patients and allocate them to a protocol that is actually followed when we apply for NIH R-01 grant for the definitive trial.
TEE Predictors of Clinical Outcomes after High Risk Surgeries
Intraoperative TEE is adopted as a routine monitor during complex high risk surgeries (e.g. cardiac). TEE provides several diagnoses and has proven utility for intraoperative monitoring of cardiovascular performance. However, studies evaluating the prognostic impact of parameters based on TEE are limited. Although some risk scores have been used to predict mortality, they include very few echocardiographic parameters. The Society of Thoracic Surgeons (STS) risk score includes left ventricular ejection fraction (LVEF) and significant valvular regurgitation or stenosis. TEE images/clips are electronically stored on all patients in centralized database and available for review and post-processing analysis by specialized Echo software . There are many parameter which could be derived from stored images on cardiovascular performance off line using QLab software. Those analysis are not done in operative room due to time constrains. The aim of this study is to retrospectively examine the parameters obtained from already stored TEE clips in complex high risk surgeries and determine which parameters predict postoperative clinical outcomes.
